    Conduct Disorders Autism redefined: Yale researchers study impact of proposed diagnostic criteria

    Getting an autism diagnosis could be more difficult in 2013 when a revised diagnostic definition goes into effect. The proposed changes may affect the proportion of individuals who qualify for a diagnosis of autism spectrum disorder, according to preliminary data presented by Yale School of Medicine researchers at a meeting of the Icelandic Medical Association.

    The proposed changes to the diagnostic definition would be published in the fifth edition of the American Psychiatric Association’s (APA) “Diagnostic and Statistical Manual of Mental Disorders (DSM-5).”

    “Given the potential implications of these findings for service eligibility, our findings offer important information for consideration by the task force finalizing DSM-5 diagnostic criteria,” said Yale Child Study Center (CSC) director Fred Volkmar, M.D., who conducted the study with CSC colleagues Brian Reichow and James McPartland.

    Conduct Disorders The underlying reason autism is often associated with gastrointestinal problems is an unknown, but new results reveal that the guts of autistic children differ from other children in at least one important way: many children with autism harbor a type of bacteria in their guts that non-autistic children do not. The study was conducted by Brent Williams and colleagues at the Mailman School of Public Health at Columbia University.

    Earlier work has revealed that autistic individuals with gastrointestinal symptoms often exhibit inflammation and other abnormalities in their upper and lower intestinal tracts. However, scientists do not know what causes the inflammation or how the condition relates to the developmental disorders that characterize autism. The research results appearing in mBio® indicate the communities of microorganisms that reside in the gut of autistic children with gastrointestinal problems are different than the communities of non-autistic children. Whether or not these differences are a cause or effect of autism remains to be seen.

    "The relationship between different microorganisms and the host and the outcomes for disease and development is an exciting issue," says Christine A. Biron, the Brintzenhoff Professor of Medical Science at Brown University and editor of the study. "This paper is important because it starts to advance the question of how the resident microbes interact with a disorder that is poorly understood."

    Conduct Disorders While pharmacologic agents have a demonstrated efficacy in children with Attention-deficit/hyperactivity disorder (ADHD), some children have suboptimal response to a single pharmacologic agent. A recent study by Dr. Timothy E. Wilens and colleagues, published in the January 2012 issue of the Journal of the American Academy of Child and Adolescent Psychiatry (JAACAP), is the first randomized placebo-controlled trial designed to assess efficacy and safety of guanfacine extended release (GXR) as an adjunct to psychostimulants in children and adolescents diagnosed with ADHD who had a suboptimal response to a psychostimulant alone.

    As reported in the article "A Controlled Trial of Extended-release Guanfacine and Psychostimulants for Attention-deficit/hyperactivity disorder," Wilens and colleagues conducted a nine week multicenter, double-blind, placebo-controlled, dose-optimization study, with participants in 59 study sites who continued their stable dose of psychostimulant given in the morning and were randomized to receive GXR in the morning, GXR in the evening, or placebo.

    Conduct Disorders Children's Hospital of Philadelphia studies common characteristics

    In the first study to investigate driving as it relates to teens with a high-functioning autism disorder (HFASD), child development and teen driving experts at The Children's Hospital of Philadelphia's Center for Child Injury Prevention Studies found that two-thirds of teenagers with a high-functioning autism spectrum disorder (HFASD) who are of legal driving age in their state are currently driving or plan to drive.

    A HFASD is characterized by subtle impairments in social interaction, communication, motor skills and coordination and by a difficulty in regulating emotions. Many of these capabilities come into play when driving.

    "Little is known about how HFASDs affect a person's ability to drive safely," explained lead author Patty Huang, M.D., a developmental pediatrician at The Children's Hospital of Philadelphia (CHOP). "Over the past decade, the rate of children diagnosed with an HFASD has increased, meaning that more of those kids are now approaching driving age. Car crashes are the number one cause of death for teenagers, so it is important that we understand how HFASDs impact driving and how to develop appropriate educational and evaluation tools."

    Conduct Disorders In the first study of its kind, researchers at Queens College and Mount Sinai School of Medicine have found that low socioeconomic status (SES) and maternal gestational diabetes together may cause a 14-fold increased risk of attention deficit hyperactivity disorder (ADHD) in six year olds.

    Led by Jeffrey M. Halperin, PhD, Distinguished Professor of Psychology at Queens College and Professorial Lecturer in Psychiatry at Mount Sinai, and Yoko Nomura, PhD, Assistant Professor of Psychology at Queens College and Assistant Clinical Professor of Psychiatry at Mount Sinai, the research team evaluated 212 children at age three or four and again at age six. They compared 115 children who had low SES, maternal gestational diabetes, or both, to 97 children who had neither, evaluating members of the control group at age three or four then again at age six. The team found that while maternal gestational diabetes and low SES increased the risk for the child to develop ADHD, the risk increased exponentially when the two factors were taken together.

    "To our knowledge, this is the first study to evaluate how prenatal exposure to gestational diabetes and low socioeconomic status together contribute to the development of ADHD," said lead author Dr. Nomura. "The results show these children are at far greater risk for developing ADHD or showing signs of impaired neurocognitive and behavioral development."
