I'm with Marg. Think of the extreme pain involved with this act. I think I'd be investigating further to make certain it was indeed a self inflicted wound. I'm not saying someone who is mentally unstable wouldn't necessarily try it, but I'd think the pain threshold of such a sensitive area would prevent them from being able to complete the act.

Or the guy had awfully good drugs.
