Doesnt your doctor have these forms at his office? Most do. Just like they have the notes to keep kids out of school if they are sick. They should have the forms for kids who need to take medication at school. I would call the doctor's office and ask them. Especially if its a psychiatrist office. They can simply fill it out and fax it to the school. If the school actually has to have a live copy, send the fax then have them mail you a copy and you can send it in with your child.
