How do you get brave enough to wean off a medication or try a new one?

And as our difficult children grow there are many times you have to changes medications due to the growth & bio chemical changes in your difficult children brain. I could never bring myself to remove a medication due to weight gain if it worked in the area it needed to, i.e. anxiety, rages, attention span, etc. , it was prescribed for.
