"Just" bugs? no. Definitely bugs, though. So thought we'd deal with that first.
Scalp has been in bad shape for a while, but it could be a number of additional factors - from lack of good brushing (as evidenced by tangles!), improper rincing, etc., to... the things HaoZi lists.

I'm going to try several ideas at the same time - treat different areas of the scalp with 2 or 3 different things, and see how it responds.

Olive oil, maybe with rosemary oil in it if I can find any.
Baking soda paste.
Vinegar... I used a vinegar/shampoo mixture when we started this whole exercise, partly to get any old shampoo out of the hair, but not strong enough to really affect scalp.
She's used ti-tree oil shampoo for most of a year, not really any effect but might not be strong enough.

Also going to switch back to baby shampoo for a bit, in case its her sensitive skin complaining about shampoo.

This is just taking so much TIME.
