Becuz doctor in the box is for acute immediate issue at hand and usually not for long term care. </p><p></p><p>when our lung capacity gets poor over a gradual time, we do not notice it in the same way as when it is a sudden acute decline like say from pneumonia. Becuz while it was reducing we were automatically adjusting to it in small steps along the way, makeing self accomodations without even being aware of it. In long term smokers, pre emphysema or emphysema are almost a given in docs eyes.</p></blockquote><p></p>
