Long long road, still not full diagnosis on 4 year old boy...

Imagine what a "normal" kid his age can say AND understand what others are saying. He may be able to hear but he probably can't understand what is being said because he hasn't learned it yet. Most kids his age have learned meanings and can understand so many things that your son is only now able to hear. He may be almost 5 but as far as language goes, he is an infant and because of his age, people expect things from him that he just cannot handle.
