[QUOTE="Signorina, post: 542848"] I am fighting with a doctor's office because they continue to bill us for a rejected claim that they submitted as a "vision test" instead of a "medical eye exam." Whenever the billing office calls me, I explain that it needs to be properly submitted as a "medical eye exam" and it will be paid. Of course, they resubmit it as a "vision test" and we are back to square one. Anyway, I was looking for a sample of a letter requesting a billing office change a medical code and came across these sample letter to insurance companies. Although the samples are in regards to eating disorder treatment, I think they could be easily edited to any mental or medical health claim/Residential Treatment Center (RTC) claim etc and thought someone might find them helpful.
