looking for info on history report about difficult child

I am hoping that someone can help me locate info that I saw somewhere on this site. I read about a detailed history that parents should write about their difficult child. This is an accurate record of everything about the difficult child that parents can take to doctors. I would like to read more about this history, but I can't find where this is on the website.
